If you’re a patient in the United States and you’ve registered your device for remediation, this site will allow you to check the status of your replacement device order. If you have not yet registered, please visit this site to start the process or call 877-907-7508. If you are a non-U.S. patient, please contact your homecare provider for updates.

On this site, you can:

  • Check the status of your replacement device order.
  • Confirm if you are matched with your durable medical equipment provider (DME).
  • Add your patient prioritization information if you haven’t done so already.

If you would like to change or update contact information (such as your address, phone number or email) or cancel your order, please call 877-907-7508.
